This information packet explains the laws of the city
of Rohnert Park (contained in the City Ordinance, Title 9, Chapter
9.80) and the policy of the Rohnert Park Department of Public Safety
pertaining to a Massage Therapist Permit. This packet also provides
instructions for completing the application and describes the various
steps necessary to apply for the permit.
Who Must Apply
Any person who performs or engages in the practice of
massage within the city limits of Rohnert Park must possess a unrevoked
permit issued by the Department of Public Safety.
General Instructions
- The address of your proposed employer must be within the corporate limits of the City of Rohnert Park.
- You must provide the following when the application is submitted:
- Copy of massage school diploma or certificate
- Copy of massage school transcript
- Proof of identity-Current-Driver's License, Passport, Armed Forces Identification Card, or similar U.S. Government identity.
- If you change employers and go to work for a different
business, your Permit is no longer valid, regardless of the expiration
date on the card. You must notify the Department of Public Safety
within 24 hours so a new permit can be issued. We will require written
verification of employment from the new employer. There is no charge
for this.
- The Department of Public Safety requires at least 14 days in which to process your application.
- You must be eighteen years of age or older.
- Permits will be issued to an individual and not to corporations, partnerships, associations, or other entities.
Applications Instructions
- Fill out the application form completely and return it to the
Department of Public Safety. Print or type all the information
requested.
- If any item does not apply to you, insert "N/A". Mark all
spaces. If you need more space than is provided, attach a separate
sheet. Show the numbers of the question you are answering and print or
type your response.
- Attach the requested documents in the same order as requested
and indicate at the bottom of the attached document the item on the
instruction sheet and/or application form to which it refers.
New Applicants
- A fee of $50.00 will be required for the city permit and must be
submitted with the application. A check or cash will be accepted. Exact
amount of cash only - no change available.
- An Additional fee of $52.00 will be required from each applicant for a life scan processing fee.
- If your permit is denied, all fees are non-refundable.
Renewals
- You are responsible for initiating a renewal application each year.
Plan to come to the Department of Public Safety approximately 30 days
before the expiration date on your current permit to begin the renewal
process.
- A renewal fee of $50.00 is required and shall be submitted with the application.
