Passports Print E-mail
The City Clerk’s Office accepts passport applications by appointment on Monday, Tuesday, Thursday & Friday during the hours of 9:00-11:00 a.m. & 1:00-3:30 p.m.  We are located at 6750 Commerce Blvd. in Rohnert Park.  Application forms are available in the City Clerk’s Office or may be downloaded from the U.S. Dept. of State official website for passport information at www.travel.state.gov.

Applying for a New Passport

U.S. Passports are issued only to U.S. Citizens or Non-Citizen nationals.  Please note that ALL applicants who do not qualify to renew by mail must appear in person.  You will need to provide the following items at the time of your appointment:

1.  Completed passport application – (Form DS-11);

2.  Evidence of U.S. citizenship - to include one of the following:

  • Certified copy of birth certificate (NOTE: photocopies of birth certificates, California birth abstracts and hospital issued certificates are not acceptable as evidence of U.S. citizenship);
  • Previous passport; or
  • Certificate of Naturalization

3.  Proof of Identity – This is usually in the form of a valid driver’s license or I.D. card;

4.  Passport photos - These can be obtained at various local retailers; and

5.  Fees – Submission of fees require two separate payments as follows:

  • A $25 execution fee for each application, payable to the City of Rohnert Park; AND
  • Either a $75 passport fee for applicants 16 years of age and over, payable to the U.S. Dept. of State; OR
  • A $60 passport fee for applicants 15 years of age & under, payable to the U.S. Dept. of State

Please note that payments to the U.S. Dept. of State must be by check, money order, or cashier's check, and a separate payment must be submitted for each application.  The City of Rohnert Park can accept payment for the execution fees in the form of cash, personal check, money order or cashier's check, and a single payment may be made for all applications.  We are not able to accept payment by credit or debit card for either fee.

Processing time for routine service is currently averaging approximately 6 weeks.  If you need to receive your passport sooner, please add $60 to the U.S. Dept. of State fee for expedite service and you will receive your passport in apporoximately 3 weeks.

Passports for applicants 16 years of age and older are valid for 10 years.  Passports for applicants 15 years of age and under are valid for 5 years.

All documentation is submitted to the U.S. Dept. of State, including birth certificates, previous passports, or naturalization papers and will be returned to you with your issued passports. 

Requirements for Applicants Under the Age of 16 

There are special requirements for minor applicants under the age of 16.  Both parents or the child’s legal guardian or guardians must appear with the child and execute the minor’s application.  If only one parent is able to appear, the second parent’s notarized written statement consenting to passport issuance must be submitted at the time of application. (Form DS-3053).

Requirements for Applicants 16 or 17 Years of Age

A minor applicant who is 16 or 17 years of age must appear in person and personally sign his/her passport application.  If payment is made with a parent's check and/or the parents are listed as emergency contacts, parental consent can be assumed.  If parental consent is not implied on the application,  it is recommended that at least one parent or legal guardian also appear with the minor child to co-sign the application.

Application for U.S. Passport by Mail

Applicants who have had a previous U.S. Passport may qualify to renew by mail if the following conditions are met:

  1. You can submit your most recent U.S. Passport;
  2. You were at least 16 years old when your most recent U.S. passport was issued;
  3. Your most recent passport was issued less than 15 years ago;
  4. You use the same name as on your most recent passport; OR you have had your name changed by marriage or court order and can submit proper documentation to reflect your name change.

If you qualify to renew by mail, you will need to fill out form number DS-82 entitled “Application for U.S. Passport by Mail” and follow the instructions contained within the application form.
