Lifesaving Information for Emergencies
During a critical illness or accident in your home, when you can't speak for yourself, when you are confused, in pain or scared, the
Vial of L.I.F.E. can speak for you. The
Vial of L.I.F.E. puts your medical information into the hands of emergency responders. This information can assist emergency personnel in administering the proper medical treatment.
The
Vial of L.I.F.E. is offered free of charge. Please
visit the Public Safety building located at 500 City Center Drive during
business hours to pick up a vial.
- Medical information form: The form provides a place for you to include information about your current medication conditions, medications, emergency contacts, insurance and hospital preferences.
- Plastic bag: Place the completed medical form in the bag and then securely tape it to the front of your fridge. The bag is large enough to hold a copy of your durable power of attorney and/or pre-hospital Do-Not-Resuscitate form.
- Sticker: Place the sticker on or near your front door or an adjacent window to alert First Responders
